What I would ask discharge planners

For a genuine “trusted by discharge planners” assessment, I'd ask hospital/SNF case managers questions like:

1. Which agencies actually respond quickly to referrals?
2. Who reliably accepts same-day or next-day admissions?
3. Which agencies communicate well after discharge?
4. Who calls the hospital/SNF when there's a change in condition?
5. Which agencies have the fewest avoidable readmissions/escalations?
6. Which agencies have dependable weekend/after-hours coverage?
7. Which agencies do your nurses/case managers personally feel comfortable referring their own families to?
8. For hospice: who is particularly strong with symptom management, family communication and after-hours response?

That last question is often more informative than asking, “Who's the best agency?”
